To our patients and partners
In connection with the entry into force of the personal data regulation in May 2018, there has also been an update of the requirements for the information that Sportsfyssen as a company must provide to the persons about whom we process information.
We have therefore drawn up an updated personal data policy for patients and business partners for whom we process personal data as covered by the Personal Data Regulation and the corresponding Danish legislation.
We typically receive information about individuals in the following ways directly from patients and insurance companies via health insurance.
We collect e.g. the following personal data:
– Name, address, phone number, e-mail address, dates of birth and possibly CPR number.
- We keep the statutory record with data about your injury and the treatments you receive from us.
- We pass on and entrust personal information to insurance companies where patients get their treatments covered, and cases where the patient himself has requested this.
– Information about CPR. no. we only pass on to authorities and business partners if this is necessary to fulfill legal requirements or requirements according to other administrative regulations.
As a rule, we pass on sensitive personal information only on the basis of express consent from the person or persons concerned to such a pass on.
According to the Personal Data Regulation, the patients for whom we process information have a number of rights.
As a person who has registered personal data with Sportsfyssen, you thus have the right to gain insight into which personal data we process about you.
By written request to Sportsfyssen, you can either receive a printout of your personal data, have your personal data updated, object to the processing of personal data or have your personal data deleted.
The request must be signed by you and must include your name, address, telephone number and email address.
You can also contact Sportsfyssen Østerbrogade 19, 1 th 2100 Kbh Ø, at mail@sportsfyssen.dk or tel. 21708633 if you believe that we are processing your personal data in violation of the regulation's provisions or other legislation.
If your objection is justified, we will stop the processing and delete your data, unless we are obliged to keep them according to the records legislation or based on the nature of the advice and related case processing we have carried out until then.
Finally, you have the right to complain about our processing of your personal data to the Danish Data Protection Authority. Such a complaint can be submitted to the Data Protection Authority, www.datatilsynet.dk, Borgergade 28, 5., 1300 Copenhagen K, tel. 33 19 32 00.
Storage safety
We aim for a high degree of security around the personal data that we process.
Security measures include, among other things, locking of premises, storage of particularly sensitive personal information in locked treatment rooms, use of recognized hosting partners with a focus on data security.
To avoid data loss, we also take daily backups of our dataset at a recognized hosting and backup center.
In the event of a security breach that results in a high risk to data subjects of discrimination, ID theft, financial loss, loss of reputation or other significant disadvantage, we will notify you of the security breach as soon as possible.
Finally, we have incorporated deletion procedures for personal data, which include personal data both in e-mails, documents, etc.
